First Visit

To save time, please fill out all new patient forms before arriving at our office.  You can complete your forms online and submit them securely to our office, or you can print them at home, complete, and bring the forms with you to your appointment.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options, as well as financial information. Occasionally, surgery can be performed the same day as the consultation, however, I.V. anesthesia, or a complex medical history or treatment plan will require an initial consultation/evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Provide a written referral slip from your referring dentist or physician
  • X-rays (be sure to bring or have your dentist send your x-rays to our office)
  • We must have a current list of all medications you are presently taking.
  • If you have medical or dental insurance, bring your insurance cards/information. 

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment and surgical treatment appointments..

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. All patients having I.V. anesthesia must not have anything to eat or drink for at least eight (8) hours prior to your surgery. You must have someone accompany you in order to drive you home safely.

You must alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves, joint replacement, rheumatic fever, bleeding disorder, seizure disorder, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anti-coagulant/blood thinner medication, bisphosphonate medication, etc.).


If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.
